Due to the involved nature of course organization, cancellation policy is as follows:

1. If registrant cancels with less than 30 days notice:
• If cancelled spot is filled with another attendee, a refund will be issued, less 10% service charge
• If cancelled spot is not filled, there will be no refund of course fee.

2. If registrant cancels with more than 30 days notice:
• Refund will be issued, less 10% service charge

3. We reserve the right to cancel the course at any time due to low attendance or other conflicts.
• Should the course be cancelled by teacher, a full refund will be issued.

I understand that completion of the P1 course does not guarantee P1 certification and that in order to obtain certification as a SSOL Schroth Physical Therapist, I must pass the SSOL P1 written exam given at the end of the course. A score of 75% is required for passing. If I fail to pass the P1 exam, I will be required to retake the exam within 3 months. If I fail a second time, I will be required to retake the course at full cost before retaking the certification exam.

I understand that SSOL P1 certification as a SSOL Schroth Physical Therapist is valid for three years and that I must attend the SSOL P2 course prior to the expiration of the three-year certification (but no sooner than one year following my completion of P1 in order to obtain permanent certification as a SSOL Advanced Schroth Physical Therapist.

I understand that failure to attend P2 in the required time frame will result in the loss of my certification, and that I will not be certified by SSOL to treat patients until I retake PP1.

I agree that I will not use the material I receive during the course to train other physical therapists in the treatment of patients with scoliosis or other spinal conditions. However, if I am a clinical instructor for physical therapy students, I understand that my students will be permitted to observe me providing Schroth therapy.

Students will be required to sign this agreement in person at start of course.

In consideration of the services of Stolze Therapies, LLC, including, but not limited to its agents, owners, officers, volunteers, participants, employees, affiliates, and all other persons or entities acting in any capacity on its behalf (hereinafter collectively referred to as "Stolze Therapies"), I HEREBY ACKNOWLEDGE AND AGREE TO IRREVOCABLY RELEASE AND WAIVE, INDEMNIFY, AND DISCHARGE Stolze Therapies, LLC OF ANY AND ALL PAST, PRESENT, OR FUTURE CLAIMS ON BEHALF OF MYSELF, MY CHILDREN, MY PARENTS, MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES/AGENTS, AND ESTATE AS FOLLOWS:


1. I ACKNOWLEDGE that I understand that physiotherapy includes physical movements, as well as an opportunity for muscular strengthening, relaxation, stress reduction, and relief of muscular tension. As is the case with any physical activity, THE RISK OF INJURY TO ME OR TO THIRD PARTIES, EVEN SERIOUS OR DISABLING, IS ALWAYS PRESENT AND CANNOT BE ENTIRELY ELIMINATED. If I experience any pain or discomfort, I will listen to my body, adjust the exercise or posture, or discontinue the exercise/posture or remainder of class, and, if necessary and available, ask for support from the teacher or staff. I will continue to breathe smoothly. I UNDERSTAND IT IS MY RESPONSIBILITY TO KEEP MYSELF HYDRATED AND PRACTICE IN A MANNER THAT RESPECTS THE SAFTEY AND INTEGRITY OF MY BODY AND MIND; THIS INCLUDES BEING PROPERLY PREPARED, RESTED, FOCUSED AND ENSURING A DISTRACTION-FREE ENVIRONMENT TO OPTIMIZE FOCUS ON SAFE FORM AND INSTRUCTIONS GIVEN DURING THE CLASS/COURSE.

2. I further acknowledge and agree that participation in this course is not a substitute for individual medical attention, examination, diagnosis or treatment. Certain exercises and information presented in this course may not be recommended, nor safe under certain medical conditions. I affirm that that I alone am responsible to decide whether to, and at what extent to participate in this course. I HEREBY AGREE TO IRREVOCABLY RELEASE AND WAIVE ANY AND ALL CLAIMS THAT I HAVE NOW OR HEREAFTER MAY HAVE AGAINST STOLZE THERAPIES, LLC.

3. I expressly acknowledge, agree, and promise to accept and assume all of the risks existing in this activity. My participation in this course is purely voluntary, and I elect to participate in spite of the risks, known or unknown.

4. I HEREBY VOLUNTARILY RELEASE, DISCHARGE, AND AGREE TO INDEMNIFY AND HOLD HARMLESS STOLZE THERAPIES, LLC FROM ANY AND ALL CLAIMS, DEMANDS, OR CAUSES OF ACTION, which are in any way connected with my participation in this course or my use of Stolze Therapies' equipment or facilities, including any such claims which allege negligent acts or omissions of Stolze Therapies, LLC.

5. Should Stolze Therapies or anyone acting on its behalf be required to incur attorney's fees and other costs to enforce this agreement, I AGREE TO INDEMNIFY AND HOLD Stolze Therapies, LLC HARMLESS FOR ALL SUCH FEES AND COSTS. I also understand and agree that I am financially responsible and will repay any attorney fees incurred by Stolze Therapies as a result of Stolze Therapies needing to enforce this Waiver.


6. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I FURTHER CERTIFY THAT I HAVE NO MEDICAL OR PHYSICAL CONDITIONS WHICH COULD INTERFERE WITH MY SAFETY IN THIS COURSE, OR ELSE I AM WILLING TO ASSUME—AND BEAR THE COSTS OF—ALL RISKS THAT MAY BE CREATED, DIRECTLY OR INDIRECTLY, BY ANY SUCH CONDITION. I understand and agree that if I have any change of medical status after signing this waiver that impacts my ability to safely participate in the exercises presented in this course, I will have received clearance from my medical doctor prior to my participation in these classes. In such a case, I understand and agree that I will take movements slowly and with awareness, listening to my body and adjusting as needed even if I did things differently before any change in status.

7. I FURTHER ACKNOWLEDGE AND AGREE that Stolze Therapies, LLC may, in its sole discretion, bar my participation in its courses in order to prevent injury or inconvenience to myself and others.

8. Photo Release: At times portions of our classes and events may be filmed, photographed, and/or recorded. By signing here, you consent to the possible reproduction of your likeness and/or voice, as well as the likeness/voice of any person representing our products and/or services, and you waive any claims in connection therewith.

I agree that I have had sufficient opportunity to read this entire document. I FURTHER ACKNOWLEDGE I HAVE READ AND UNDERSTOOD THIS STUDENT RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, AND I AGREE TO BE BOUND BY ITS TERMS.
